Case Managers/Marketing

A Case Manager plays a crucial role in coordinating and overseeing the care of individuals in various settings, such as healthcare, social services, mental health, or rehabilitation. Their responsibilities generally revolve around assessing client needs, developing care plans, and ensuring that clients receive the necessary resources and support services. Case managers are responsible for advocating for clients and facilitating communication among healthcare professionals, service providers, and clients themselves.

  • Gather Information: Collect comprehensive data from clients, family members, healthcare providers, or other agencies to understand the scope of care required.

  • Set Goals and Objectives: Collaborate with the client to establish clear, realistic short-term and long-term goals for their care or rehabilitation.

  • Regular Monitoring: Follow up with clients to monitor their progress, check if they are meeting care goals, and assess any changes in their condition or situation. This might involve regular phone calls, home visits, or visits to healthcare providers.

  • Link Clients to Community Resources: Case managers connect clients with resources available in the community, such as support groups, educational programs, transportation services, food banks, or housing programs.

  • Community Outreach: Promote awareness of available services within the community and help clients access the support they need, particularly when those services are outside the organization or healthcare setting.
